Toxic, Narcissist आणि Personality Disorder: आपण माणसांना समजून घेतोय की फक्त त्यांना Label लावतोय?

काल सहज बाहेर पडलो आणि एका नवीन चित्रपटाचं poster दिसलं Toxic. शब्द नजरेत पडला आणि अचानक मला एक वेगळाच प्रश्न पडला : हा शब्द English भाषेत आला तरी कुठून?

Toxic हा मुळात काही psychology मधून जन्माला आलेला शब्द नाही. त्याचा संबंध toxicity, toxic substance, poison, intoxication अशा वैद्यकीय आणि रासायनिक संदर्भांशी आहे. म्हणजे एखादा पदार्थ शरीराला विषारी ठरतो, शरीरात विषारी परिणाम निर्माण करतो, त्याला toxic म्हणता येतं.

पण मग हळूहळू भाषेने एक मोठी उडी घेतली.आता toxic हा पदार्थासाठी कमी आणि माणसांसाठी जास्त वापरला जाऊ लागला.एखादी व्यक्ती आपल्या अपेक्षेप्रमाणे वागत नाही toxic.एखादा मित्र आपल्याला विरोध करतो toxic. एखाद्या नात्यात मतभेद झाले toxic relationship. एखादी व्यक्ती आपल्याला आवडत नाही toxic person.आणि सर्वात मजेशीर भाग म्हणजे आपण हा निष्कर्ष इतक्या conviction ने सांगतो की जणू त्या व्यक्तीची laboratory test करूनच आलो आहोत.

"तो माणूस toxic आहे."   कशावरून?

"कारण तो माझ्या विचारांशी agree करत नाही."

"ती toxic आहे."   का?

"कारण मला तिच्याबरोबर राहायला आवडत नाही."

मला हळूहळू लक्षात आलं की toxic हा शब्द अनेक वेळा एखाद्या व्यक्तीचा objective गुण नसतो; तो आपल्याला त्या व्यक्तीसोबत आलेल्या अनुभवाचा subjective निष्कर्ष असतो.

आणखी गंमत म्हणजे एखादी व्यक्ती आपल्याला सहा महिने अत्यंत चांगली वाटते, आणि सातव्या महिन्यात आपली इच्छा पूर्ण केली नाही की तीच व्यक्ती अचानक toxic होते. म्हणजे toxicity बदलली की आपली convenience?

मी psychology कडे वळलो. आणि इथे तर शब्दांची गर्दी अजून मोठी....

Narcissist.
NPD.
BPD.
ASPD.

Histrionic Personality Disorder. आजकाल एखाद्या माणसाला भेटा आणि दोन conversations झाल्या की diagnosis तयार. भेटायची तसदी न घेता काही हुशार लोक, प्रत्येक्षात कधीही न भेटलेल्या व्यक्तीला 'toxic' किंवा 'asocial', 'antisocial' ठरवून मोकळे होतात.

"तो narcissist आहे."  "ती BPD आहे."

"तो ASPD असावा."  "ती Histrionic आहे."

मी विचार करू लागलो—आपण खरंच psychology समजून घेतोय की psychology ची शब्दावली वापरून आपल्या personal judgments ना authority देतोय?

एखाद्या व्यक्तीला दोन-चार behaviours पाहून या disorders पैकी कोणताही label लावण्याचा अधिकार किंवा वैज्ञानिक आधार सामान्य माणसाकडे नसतो.आणि ही दोन वाक्ये एकमेकांपासून वेगळी ठेवली नाहीत तर आपण psychology चं अतिशय मोठं नुकसान करतो.

मी Cleveland Clinic वर याबद्दल वाचत होतो.

Attention ची गरज, emotions ची dramatic expression, approval ची इच्छा, इतरांच्या प्रतिक्रिया यांच्याबद्दल sensitivity अशा अनेक गोष्टींचं वर्णन तिथे आढळतं.

आता कल्पना करा की एखादी व्यक्ती social gathering मध्ये खूप बोलते, सगळ्यांचं लक्ष वेधून घेते, colourful कपडे घालते, भावना उघडपणे व्यक्त करते आणि praise आवडतो.

आपण लगेच म्हणू शकतो "Histrionic!"

पण थांबा.आपण personality disorder diagnose केला की फक्त काही personality characteristics पाहिल्या?

कारण दोन्ही गोष्टी सारख्या नाहीत. Expressive असणं म्हणजे Histrionic Personality Disorder नाही. Attention आवडणं म्हणजे disorder नाही. Fashionable कपडे घालणं म्हणजे disorder नाही. Emotional असणं म्हणजे disorder नाही.

लोकांना प्रभावित करण्याची इच्छा म्हणजे disorder नाही. Clinical diagnosis साठी pattern, persistence, context, impairment आणि एकूण psychological functioning पाहावं लागतं. 

मग मला एक वेगळी गोष्ट जाणवली.आपण एखाद्या माणसाचा behaviour पाहतो आणि त्याच्या behaviour चं कारण स्वतःच ठरवतो.

एक माणूस सतत स्वतःबद्दल बोलतो. आपण म्हणतो Narcissist. ,पण तो anxiety मुळे स्वतःला prove करण्याचा प्रयत्न करत असेल? , तो profound insecurity मध्ये असेल?. तो एखाद्या mood episode मध्ये असेल?, त्याला rejection ची तीव्र भीती असेल?, किंवा त्याच्यात pathological narcissistic traits असतील.

पण बाहेरून दिसणाऱ्या एका behaviour वरून यापैकी कोणतं कारण खरं आहे हे कसं ठरवलं? Research मध्ये narcissism हा एक simple "आहे किंवा नाही" असा switch नाही.

Krizan आणि Herlache (2018) यांनी narcissism चा Narcissism Spectrum Model मांडताना narcissistic phenomena कडे अधिक dimensional पद्धतीने पाहण्याचा प्रयत्न केला.

म्हणजे grandiosity, antagonism, vulnerability यांसारखे वेगवेगळे dimensions आणि त्यांचे परस्पर संबंध.

हे वाचताना मला एक गोष्ट प्रकर्षाने जाणवली.Internet वरचा narcissist अत्यंत सोपा आहे😀.

Research मधला narcissism अत्यंत गुंतागुंतीचा आहे.

Internet म्हणतं "He doesn't apologise. Narcissist."

Research विचारतं  "हा pattern किती persistent आहे? कोणत्या contexts मध्ये दिसतो? त्यामागे कोणते traits आहेत? इतर psychopathology आहे का? Functioning किती impaired आहे?"

Borderline Personality Disorder.

हा label तर काही लोक इतक्या सहजपणे वापरतात की एखाद्या व्यक्तीच्या emotional reaction ला diagnosis मिळतो.

Mood बदलला? BPD.

Attachment जास्त आहे? BPD.

Breakup नंतर emotional झाला? BPD.

Anger आला? BPD.

पण emotion ची intensity आणि personality disorder ही समान गोष्ट नाही.

मानवी भावना psychiatric disorder नसतात.

आणि कदाचित आजच्या काळात सर्वात धोकादायक psychological disorder हा एखाद्या textbook मध्ये नसून आपल्या विचार करण्याच्या पद्धतीत आहे

आपल्याला अपूर्ण माहितीवर पूर्ण certainty हवी असते.

आपल्याला behaviour नको; explanation हवी.

आपल्याला complexity नको; label हवा.

आपल्याला "मला माहीत नाही" म्हणायचं नसतं.

म्हणून आपण म्हणतो 

Narcissist.

Toxic.

BPD.

ASPD.

Histrionic.

आणि मग स्वतःच्या निष्कर्षावर इतक्या conviction ने विश्वास ठेवतो की दुसऱ्या व्यक्तीला स्वतःचं स्पष्टीकरण देण्याची संधीही राहत नाहीTheodore Millon यांचं Personality Disorders in Modern Life वाचताना personality pathology किती complex असू शकते याची जाणीव झाली.तसंच E. Ronningstam यांच्या Narcissistic Personality Disorder: A Clinical Perspective मधून narcissistic pathology कडे clinical दृष्टिकोनातून पाहिलं की internet वरचा "narcissist" किती caricatured आहे हे स्पष्ट होतं.

Pincus आणि Lukowitsky (2010) यांनी pathological narcissism आणि NPD वर लिहिताना narcissism च्या complexity कडे लक्ष वेधलं. म्हणजे clinical literature म्हणतं "Understand the structure." Social media म्हणतं "Spot the signs."😁

मला वाटतं या दोन गोष्टींचा फरक आपण लक्षात ठेवायला हवा.

psychology चा  सर्वोच्च टप्पा म्हणजे लोकांना label करण्याची क्षमता नव्हे; लोकांबद्दल निष्कर्ष काढण्यापूर्वी स्वतःच्या मनाला थांबवण्याची क्षमता.

आणि कदाचित खऱ्या अर्थाने intelligent माणूस तोच

जो प्रत्येक कठीण व्यक्तीला "Narcissist" म्हणत नाही, प्रत्येक दुःखी व्यक्तीला "BPD" म्हणत नाही, प्रत्येक संशयी व्यक्तीला "ASPD" म्हणत नाही आणि प्रत्येक dramatic व्यक्तीला "Histrionic" म्हणत नाही.

Buddhist Diagnosis of Human Suffering

  Buddha’s diagnosis of human suffering has two parts in which he explains the  nature of suffering and the cause of suffering , contained in the First and Second Noble Truth respectively. The First Noble Truth explains the existence of suffering, referring to it with the word dukha, which encompasses the underlying sense  unsatisfactoriness of human experiences and its pervasiveness throughout life . It also points towards the impermanent nature of reality, namely pleasure. Another aspect of duhkha besides the pursuit of pleasure is the aversion of pain and suffering.
